Vision Children Registration

This form must be completed by a parent or legal guardian.

By submitting this form, I agree:

For my child to attend Vision Children at Vision Church;
That it is my responsibility to ensure my child is delivered to and picked up from all Vision Children rooms by an adult, and signed in and out (Creche and Preschool - Year 2);
For his/her personal details to be on the Vision Children database, for use by the Children’s Ministry Leaders. I understand the information I provide concerning my child will be kept confidential and only shared with relevant leaders.
For Leaders to take photos or video of my child as a record of various activities undertaken by Vision Children and when necessary, to keep on file and to show during church services/activities (photos and video will not be livestreamed to the internet);
I agree that I may be placed on a roster unless otherwise arranged with room coordinator and understand I need to hold a Working With Vulnerable People (WWVP) card.
I am aware of the Vision Church Safe Ministry Policy.
