By submitting this form you agree: For your child to attend Vision Youth events. You understand that it is your responsibility to ensure that your child is delivered to and picked up from Vision Youth events; For Leaders to arrange for your child to receive any such medical treatment deemed necessary, and if they are unable to contact you, you also authorise the use of ambulance if necessary and accept responsibility for all expenses associated with such treatment; For your child's personal details to be on Vision Youth database, for use by the Youth Ministry Leaders; For Leaders to take photos or video of your child as a record of various activities undertaken by Vision Youth and when necessary, to keep on file. You agree to photos being used on the Vision website or Instagram page. Individuals will not be identified. Photos and video will not be livestreamed to the internet; For your child to travel by car during Vision Youth events, driven by a fully licensed driver approved by pastoral leadership; I am aware of the Vision Church Safe Ministry Policy.
